Mail your completed rebate form and all proofs of purchase required to the address given on the rebate form.
Allow at least 6 - 8 weeks for your submission to be received, processed and mailed back to you. If it has been 6 weeks since you mailed your submission, and you have not yet received your rebate, go to the
Check Your Rebate Status section of this website.
